Health Forms: The client will be required to complete a confidential health form prior to a table massage and an abbreviated release form prior to chair massage. This is to ensure no medical conditions exist which would prohibit massage therapy.
Licensing: The State of Minnesota does not currently license massage therapy. Some cities have ordinances regulating the practice of massage therapy. Prior to scheduling on-site services, Snyder Massage and Bodywork, LLC will contact the city for their regulations.
Sexual Harassment: A client can expect all services to be performed in a way that is courteous and free of abuse by verbal, physical, or sexual means. Any illicit or sexually suggestive remarks or advances made by the client will result in immediate termination of the session and the client will be liable for payment of the appointment.
Illness: If the client has a cold, flu, or other contagious illness, it is requested the appointment be rescheduled. Snyder Massage & Bodywork, LLC reserves the right to deny massage and bodywork should a client present the above conditions.
Cancellation: A 24-hour cancellation notice is requested. A prepayment for future appointments may be requested if the 24-hour cancellation policy is not observed.
Appointment Time: Because another client may be scheduled before or after an appointment, sessions will begin and end on time. If, for example, a client schedules a 1 1/2 hour massage and is half an hour late, the client will be charged for the full 1 1/2 hour appointment.
